Veterans Benefit Request UMD

This form collects your enrollment information and veteran benefit program(s) you are requesting participation in. 

  • Your VA file number is required in order to certify your enrollment with Veterans Affairs. If University Veteran Service (UVS) does not have your file number, please send this information to them at [email protected]
  • If this is the first time you will be using your benefits here at the University of Minnesota Duluth, then you are required to contact University Veteran Services (UVS) in order to go over the necessary information/documentation.
