This form is an application for current University of Minnesota Duluth students who want to change their current college to another UMD college or want to apply for more than one degree (i.e., BA and BS).
This form can also be used for students who are currently in more than one degree and want to drop one.
Before you complete this form, discuss your interest in changing colleges or adding an additional degree with your academic advisor.

Use this form if...
- You are a currently enrolled UMD student and want to change your current college to another UMD college.
- You are a currently enrolled UMD student and want to add an additional degree (i.e., BA and BS).
- You are a currently enrolled UMD student with more than one degree, and you want to drop one.
Don't use this form if...
- You have not yet started your first semester at the UMD campus or are transferring from outside the University, you should contact the Office of Admissions for assistance.
- You are adding an additional major to your same degree program. Please consult with your Collegiate Advising Office for assistance. For example, if you are an Economics B.A. major, and you want to add a Biology B.A. major, that is a second major in your same degree program. Therefore, please go to your Collegiate Advising Office instead of completing this form.
