Student Services Fee Assessment Request

This form collects information on the term you are requesting, and the cost of the Student Services Fee and fees associated, you are electing to pay. 

  • You must be enrolled for at least one (1) credit or enrolled for Grad 0999 and have no financial holds on your student account to request the assessment of the Student Services Fee and fees associated.
  • Electing to pay the fee does not automatically allow you to be eligible for the Student Health Benefit Plan.

Use this form if...

  • You want to pay the Student Services Fee and fees associated, to use or support the services covered within said fees.

Don't use this form if...

  • If you are enrolled in the Senior Citizen Education Program, Regents’ Scholarship recipients, or Academic Staff Tuition Benefit recipients.
