An incomplete “I” grade can be assigned at the discretion of an instructor. Incompletes may be assigned when extraordinary circumstances prevent a student from completing coursework on time.
This form is a contract between a student and instructor that outlines the requirements to complete the class.
You should initiate this contract after a conversation with your course instructor about completing a class with an Incomplete Grade. You should either have an “I” grade (incomplete) or no grade assigned for the class to complete this form.
Use this form if...
- You and your instructor have agreed to an incomplete grade
- You are an undergraduate student
- You are a graduate or professional student, and you have been instructed to complete this
Don't use this form if...
- You and your instructor have not agreed to an incomplete grade
- You are disputing a grade received in your course
- You are a graduate or professional student, and your department or college has its own process for incomplete contracts
