This form is used when additional documentation is required to support responses provided on the Free Application for Federal Student Aid (FAFSA) regarding your homelessness situation.
- You are required to provide a letter of determination, on letterhead, from one of the following:
- The director or designee of an emergency or transitional shelter, street outreach program, homeless youth drop-in center, or other program serving those experiencing homelessness
- Your high school or school district homeless liaison or designee
- The director or desginee of a project supported by a federal TRIO or GEAR UP program grant
- If you do not have a liaison or designee, and believe you are an unaccompanied youth who is homeless, please contact One Stop Student Services and request an appointment with the Unusual Circumstances Appeal counselor

- You have not selected any of the homelessness options on the FAFSA
