FAFSA Student Personal Circumstance Certification

This form collects additional information needed to determine your dependency status as reported on your FAFSA, in order to calculate eligibility for financial aid programs.

Documentation supporting your answers is required, to submit this form you will be required to upload the following:

  • Documentation of your dependents and your support of them
  • Documentation to reflect your orphan status
  • Foster care documentation from your social worker or a court of law
  • Ward of the court documentation from a court of law or a letter from your social worker
  • Legally emancipated minor documentation from a court of law in your state of legal residence
  • Legal guardianship other than your parent or stepparent documentation from a court of law in your state of legal residence
