Denied Credit for Federal Direct PLUS

This form collects the signature of your parent to confirm they have been denied credit for a Federal Direct PLUS (Parent) loan and they will not be pursuing an endorser for credit approval. Signing this statement will allow the office of Financial Aid and Scholarships to offer the student an additional Federal Direct Unsubsidized Loan, once all information is received.
