This form collects information for undergraduates adding a class, changing sections, or changing the credit value of a variable credit course.
If you need to add a full-semester length undergraduate class(es) after the end of week 10 of the semester:
- Obtain approval via UMN email from the instructor of the class you are adding
Use this form if...
- You are changing from one section of a class to another section of the same class after the second week of the semester through week 10
- You are changing the credit value of a variable credit course after the end of the second week of the semester
Don't use this form if...
- You are a graduate student
- You need to add a full-semester length undergraduate class during weeks 1-10, you can do so through your MyU Shopping Cart as long as the instructor/department has entered permission
- You need to change the Grading Option (A-F, S/N) after the second week. That request needs to be on an Academic Petition form
