When you fill out this form, indicate the program to which you want the credits applied. An advisor must approve that the credits to be transferred are eligible to be used toward the graduate degree.

- Credits may not be used twice, except in the case of Regents approved joint/dual programs
- Classes must be completed and graded
- Student must already be admitted to the target career
