This form collects information needed to determine your eligibility to reinstate financial aid after earning a bachelor’s degree.
- To be eligible for federal student loans, you must be admitted to and enrolled at least half-time in an eligible undergraduate or graduate degree program or licensure program. Only required credits will be accepted for the payment of financial aid
- You may be eligible for federal student loans for one 12-consecutive-month period, with half-time or greater enrollment status, if you are required to complete prerequisite course work for admission to a graduate, or professional program at any accredited college or university. Only required credits will be accepted for the payment of financial aid
- Students who are enrolled at less-than-half-time status and meet these petition requirements may need to complete this form in order to borrow from private education loan programs that lend to less-than-half-time students
- A list of courses needed to complete your program is required if you are completing a second degree or licensure program or if you are completing prerequisite coursework; your college student affairs/advising office will submit this. Please ensure you have completed any advising requirements prior to submitting this form
